Verification Procedures
Licenses do not need to be current to qualify for credit. However, whether the license is current may affect how a student will be able to obtain official documentation:
- If a student has a current license, or obtained a license fairly recently, students should be able to verify the license online and submit a screenshot to admissions@sunyempire.edu.
If online verification is not possible, students must request a Certificate of Licensing Status from the NYS Insurance Department. Students should, upon receipt, send the certificate sealed in the original envelope to the Admissions office. If the original envelope has been opened, the student should include it with the certificate when forwarding to Admissions.

Students may also fax their request for a Certificate of Licensing Status to 518-474-5048. Faxed requests must include the following information: Name, Date of Birth, License Number, and the address for the Empire State University Admissions Office.
Evaluation Info
This evaluation applies to exams completed during a specific time frame. Refer to the "Begin" and/or "End" dates listed above the credit recommendation for details. Please note that credit recommendations and minimum acceptable scores may be different for other time frames.
The information contained in this equivalency is unofficial. Eligibility for this credit must be confirmed directly with the SUNY Empire Office of the Registrar.

Students must have official documentation on file with the university to be eligible for credit. PLEASE NOTE: To be considered official, the source institution (specified in the verification procedures on the left) must send documentation directly to Empire State University.
