Former CVS employees can order documentation using the following steps:
- Call 888-myHR-CVS to request an official letter and transcript verifying completion of your training.
- Submit a copy of the unopened letter and transcript to the SUNY Empire Admissions Office.
Please note that students must successfully complete all sections of the training program to be eligible for credit.

This evaluation applies to exams completed during a specific time frame. Refer to the "Begin" and/or "End" dates listed above the credit recommendation for details. Please note that credit recommendations and minimum acceptable scores may be different for other time frames.
