Students may request official official documentation either through mail or email.
By mail, students can submit a "request for training information" form to the following address:

Albany, NY 12210- By email, students can request training and registry information from OPS.RecordsMgt@dcjs.ny.gov. Please include the following information:
- Brief description of the request
- Requestor's full name
- Last four digits of the Social Security Number
- Date of birth
- Name(s) or employer(s)
Students should then forward the training record in the original, unopened envelope to the SUNY Empire Admissions Office.
Evaluation Info
This evaluation applies to exams completed during a specific time frame. Refer to the "Begin" and/or "End" dates listed above the credit recommendation for details. Please note that credit recommendations and minimum acceptable scores may be different for other time frames.
